Appointment Information:
Please allow 2 – 3 hours for your first appointment. All new patients will need to complete a New Patient Form on arrival.
What to bring if you have them:
- Medicare card
- Referral Letter
- Current spectacles (if you have them)
- Latest spectacle and/or contact lens prescriptions if you have them handy
- Any eye drops and medications you are currently using
- A driver to take your home
- Private health insurance card as we provide HICAPS services if needed
